To clarify other situation:
When there is one lady: We title: :"Madame" eg. "How are you madame?"
When there are two ladies: We title them "ladies" eg. "How can I help you, ladies?"
When there is one gentleman: We title him "sir" eg. "How may I help you, sir?"
When there are two gentlemen: We title them "gentlemen" eg. "What can I do for you, gentlemen?:
When there is one guy (very formal): eg. "Whats up mate'...?"
When there is one girl: eg. "How are you doing miss...?"
When there is a group of rather younger people, we go: eg.: So what will it be, guys?
But when there is a an older couple / or a women and man in one group/or just a group of mixed sexes: We title them formaly: ?????????????

This is not formal....this is a very informal, casual greeting. Basically British or Aussie in nature.

When there is one girl: eg. "How are you doing miss...?"

How do you know she is unmarried??? You will insult her if she is. Even if she isn't married, many women object to being called "Miss".

A generic greeting that can be applied to everyone would be

How is everyone doing? or How are we doing? (plural)

How are you doing? (singular)
