"in" is more common but it does not imply it is always correct.it depends on the context.
If I say something with a loud voice it means I believe it is an important thing which should bot be ignored.
People were talking in a loud voice means they were talking loudly (or even shouting their viewpoints).

How about: "She shouted his name at the top of her voice"=extremely loudly
