This is when I'm not sure the exact day but somewhere between 15~19th of the month (as in youngesters in that age range "late teens"). I have a feeling native speakers don't say that right?

No - we don't use "teens" in this way.
We would say "I would like to verify if my appointment is in the third week of this month" or would specify the dates.
