It means having to work extremely late at night, or extremely early in the morning. This may be because you are scheduled to work at this time, or because you have a lot of work to catch up on.
Hospital staff in the UK often have to 'work ungodly hours'. The idea is that they work excessive hours, and often throughout the night.
