Is it correct to use "referred" with "to" when making reference to a previous document, i.e., "as evidenced in the referred to document" or should it be "as evidence in the referred document" without "to"?

Try replacing it with "referenced" instead.
As described in the referenced letter.
Of the two, "referred to" sounds better to me. This is the letter I referred to. This is the referred-to letter. But This is the letter I referenced and this is the referenced letter both sound better.
Individual taste? Maybe.

Barb_D is absolutely right in her suggestions.
Mine would be as follows:
...in the referred-to document. or ...in the document referred to.
