The correct sentence is "this forum has driven me mad".
The correct form is driven because it is a past participle (third form). We used past participles with the auxiliaries "have" and "has" to form the present perfect. And we use "had" with past participles to form the past perfect.
You could say "this forum drove me mad". This is simple past.
