The name of the discipline is 'mathematics' so a tutor of that subject would be called 'a mathematics tutor'.

I guess our dear Bhai could shed some light on this:
Jasum, you also could say:
Math(s) teacher.
I am not sure, I guess 'Math' is used mostly in AmE.

Yes, in BrE it's 'maths teacher'.
