It has to be "mistaken". It's incorrect to use "mistook".

Can we also use Mistook here in this sentence if we change the sentence formation and structure?
Like we're in a world, where the signifier has mistook the signified.

No
has or have must be followed by a past participle
so,
"has mistaken" is right.
but I don't think that it could be used in your sentence. It doesn't have the same meaning of the original sentence.
