The dictation says that the patients last visit was a year and a half ago.
I did not use any hypens for a year and a half ago. What are the rules for
hypenation here? It is not a 1 1/2 year old visit -- have already been there!

There is no need to hyphenate unless it is an adjective, Ritaivan.
'The patient's last visit was a year and a half ago.' -- this is fine.
'A year-and-a-half-year-old child fell into a well yesterday.' -- now we need hyphens.
