"Vans" would seem to be the name of a shoe company, so I would imagine it means "I'm wearing my "vans" shoes". Look here: Vans Shoes - Official Site

The verb is "to mean" the simple past and past participle is "meant", "meaning" is the present participe/gerund form. As with all English verbs, it takes an "s" in the third person singular, he/she/it, there fore we get "she/he/it means".
