(NOT a teacher) He has only a few friends, _____? Here in the United States, most people would use "doesn't he." "Hasn't he" is also correct and preferred in some other countries.

In non-assertive contexts, 'have' can function as an auxiliary (formal) or a lexical verb (formal, informal).
Has he not (only a few friends)? -- (the first auxiliary (operator) 'have' starts the sentence)
or
Does he not (have only a few friends)? -- ('does' support operator starts the sentence)

Because the statement is "he has only a few friends" and logically the tag is "hasn't he". I can't imagine why anyone would want to introduce "does". Merry Christmas to you too.

The answer is very simple. Have, be, and do are primary verbs. In non-assertive territories (negatives and questions), 'have' can be either an auxiliary or a 'normal' verb. If we treat 'have' as an auxiliary, then it will be the operator that introduces the question. In case we regard it as a normal verb, do support is needed.
