Default "a/an" instead of "the" before first/second/third,etc?

In what situation do we use "a/an" instead of "the" before first/second/third,etc?Could you please give me some examples?Thank you very much.

'A first' can be used as a pharse on its own:

'I've never done that before.'
'Yes, it was a first for me too.'
