    keep an eye on = watch carefully/look after

    The teacher kept an eye on the students during the test so they wouldn't cheat.
    Could you please keep an eye on the baby while I go to the store?

    keep an eye out for = watch carefully because you want to notice or find something

    Mary had lost an earring, so she kept an eye out for it when she swept the floor.
