Can someone help me with my letter for school. I have to put periods, etc & I'm having a hard time.

This is a evaluation of a 46 year old woman weighing 116 Pounds standing at 5ft 3Inches tall. She is post menopausal small salutation. Has a family history of osteoporosis current therapy includes regular weight bearing exercises calcium submination vitamin d and hormonal replacement therapy. The lumbar imagines of good quality. the patient has normal bone mass. Regular testing is not necessary for the next 5 years if she continues with the above treatment program. please do not hesitate to contact me if you have additional questions about this evaluation.


Please help me this needs to be in a letter form.

Gill hardly makes any error and I have learnt a lot from him. I beleive, he might have just missed during the correction. I just wanted to point that out.
