Default transitional words and transitional phrases

Every I look for information about transitional words and transitional phrases inside grammar books, they only give me a list of examples instead of explaining the general format of what makes words and phrases transitional. I would like to be able to recognize what is transitional instead of memorizing a whole bunch of them.

Can anybody please help me with my two little transitional problems?
