A woman got into the van. She sat in the passenger seat.
The driver asked her what happened (because she was very sad).
He asked: are you all right?
She started and looked at him "What? Yeah, I'm good".
Does it mean she started the car sitting in the passenger seat?

No. it means she made a sudden movement as if surprised or shocked.

She started and glanced over him,'What!Yeah, I'm good'.

As Rover said, it is. However, I think it would be more natural to say, "she glanced over at him".
