They mean different things. The first is asking about the boy's name and the second is asking about the boy naming something, or writing down his name. Have you looked up "have" and "put" in a dictionary?

Are they questions? If so,
Q: What name would you like the boy to have?
A: I would like the boy to have the name Mark/to be called Mark.
Q: What name would you like the boy to put (down in writing)?
A: I would like the boy to put (down in writing) his full name.
