Strict rule for saying to tell?

Is the following conversion so strict? Is it always converted to 1 instead of 2 or any other? I mean converting direct speech to indirect speech.

ex)He said to me, "You look pale. Are you sick?"
1)He told me that I looked pale, and asked me if I was sick.
2)He told me that I looked pale, and if I was sick(or Are you sick?)

#2 is not possible. You cannot tell someone a question.
