The phrase "not all it's cracked up to be" means that it doesn't live up to its reputation. I don't know the whole context here but Jim is implying that Melinda must be enjoying her time at the hospital, "in hog heaven" is a largely US expression meaning happy or content in a situation.
Melinda is saying that it's not as good as you might expect.
