I'd use them like this, though others may see things differently:

1 I don't think that will happen
2 Pretending to be shocked
3 Sarcastic way of pretending to express surprise

As an American English speaker, I have not seen and would not use #2.

The same for me, a speaker of BrE. I would be able to guess its meaning if it came up in a third-rate sitcom.
