I chose B but the answer is C. Why?
Do you mean answer C (Are you doing OK?) is correct? because "how do you do" is not a question but a greeting .
"How do you do" is a question but over the years the meaning has been understood as a greeting.
"Charlie, I would like to introduce you to my boss"
"How do you do? My name is Joe, I am the manager of this plant."
"How do you do" is not used very often except on formal occasions.
