Number 1 is a bit wordy and you had a missing word (which I've added). Number 2 is comprehensible, yes, but I'm not sure it's the best way to word it.

I know the sentence is talking about aging.
If this is the case, shouldn't we say 'going downhill physically'?

I would take the sentence to mean either aging, or worsening in health (yes, probably due to aging!) and "physically" would make it clearer but I don't think it's completely necessary. The context of the piece probably helped.
