"It sometimes does not add a site because it was failed to modify the the file on the server."

The good news: I think I understand the meaning. The bad news: It is not correct.

Maybe: Sometimes it doesn't add a (web)site, because it doesn't update the file on the server.

You might also mean:
"It sometimes does not add a site because the file on the server was not modified."
"It sometimes does not add a site because the file on the server has not been modified."

That is, the responsibility for modifying the file was not 'its'; it was someone else's, maybe yours.
