Mad =mentally ill or angry?
You can't know without context.
In the US, we are more likely to mean angry than crazy, but you still need context.
I'm not a teacher, but I write for a living. Please don't ask me about 2nd conditionals, but I'm a safe bet for what reads well in (American) English.
As Barb D said, without context it's impossible to tell.
In BrE, it's more likely to mean "mentally ill" but "mad" is being used more and more often here to mean "angry" too.
