    Ask the patient to book the appointment by herself.(Is it incorret?)

    Ask the patient to book the appointment herself.

    Which one is correct? And what is the difference.

    by herself = alone/without any help
    herself = she does it not someone else

    It is acceptable grammatically, though we are unlikely to want/need to say it.
