Do I need the word "for" in this sentence?
I'd like to request the following materials.
OR
I'd like to request FOR the following materials.

No, you don't need "for" after "request" when it's used as a verb. Your first sentence is correct.
However, if you use "request" as a noun, then you would need "for".
I would like to make a request for the following materials...
