From an episode of Pete & Pete "Space, Geeks, and Johnny Unitas", Pete says his science teacher is a real 'ear-gouger' while you see the guy (the teacher) poking his earhole with the finger.

What is ear-gouger?

He is speaking about his teacher. Or listening to his teacher, to be more exact. He means he would rather gouge out his ears than listen to the teacher lecture.

This is exaggeration, of course. He hasn't really gouged his ears out.
