    Default Question mark

I am not sure about the usage of question mark in situations like the following.

Could you also forward this invitation to Prasad, I do not have his email address?

    Is this sentence grammatically correct?

If you start with "Could you ..." then it should end with a question mark. However, as you have made the two sentences run together, it does look odd to have the question mark after "I do not have his email address".

    I would suggest:

Could you also forward this invitation to Prasad? I don't have his email address.

"Could you also forward this invitation to Prasad." is right without a question mark.
Do not use a question mark with a polite or formal question.

I'm sorry but I have to disagree. "Could you" is quite clearly the start of a question and therefore a question mark is entirely appropriate. If it said "Please forward the invitation to Prasad", no question mark would be necessary.
