It can mean either - and often there is no difference.
2)Does I it mean they don't apply in normal playing or that it they appliesy but on a few occasions?It means that they do not often apply - i.e, they apply, but on few occasions.
This is the sort of question that you should be able to answer from your own knowledge of the game.
Context is always important; labelling is rarely important.
