I agree. Unfortunately, news agencies have cut their staff significantly in recent years in an attempt to save money, so a lot of material doesn't get properly copy-edited before publication.
The parties involved were probably not too concerned about the superfluous comma.
I love one line from the report: The investigator said he's never seen an incident like it before.
I rather doubt that any of us has.
Context is always important; labelling is rarely important.
