Default Crediting things

Hi this is my first time to credit someone but i dont know how it's properly done. I made a cd presentation for assignment purpose and i used their images & video. What format do you use when you credit someone in a CD-Rom presentation? This is my guess i wrote:

I would like to credit the following elements:

All Images taken by http://www.dolphins.org/
All Videos taken by http://www.dolphins.org/
Layout designed by Me
Structured designed by Me

If you have had to obtain copyright permission, then use someting like 'courtesy of'
