I've recently read a book where somebody said:the icy blue eyes that had looked at him for all these years and thad had known him as a child.....
Shouldn't that be either: the icy blue eys which had looked at him all these years and which had known him as a child
or:The pair of icy blue eyes that....? Thanks or your help,

It's simply a typo. 'thad' should be 'that'.

Why do you think it should have read "which" instead of "that"?
