Which of the following sentences can describe "2+2=4" ? Are they all correct?
A. Two plus two equals four.
B. two plus two is equal to four.
C. Two plus two equates to four.

All of them could work, but A is the most common. You might also hear B. However, I doubt you'll see it expressed as C very much, if at all.
You'll also hear:
Two and two is/are four.
Two and two makes/make four.
Context is always important; labelling is rarely important.
