    Hi,

    1) If you even think you are having a heart attack, call 911.

    2) If you only want romance, make a date to see a different film.

    Could you please check if it is natural or not? And the second sentence doesn't refer to 'if only/wish', is that right?

    These both look OK.
    That's right, it means: "If you want romance (and nothing else), make a date to see…".
