Default use of "a" and "an" with abbreviations

Which whould I use in the following:

A Licensed Vocational Nurse (LVN).
A LVN or an LVN? Which is correct and why?

Pronunciation is the key.

a CIA agent ("C" is pronounced [si:]. CIA is consonant-intial)
an NDP leader ("N" is pronounced [en]. NDP is vowel-initial)

Note, "e" as in bed.
