    Default medicine vocabulary

    Hi,

    I'm translating something and I have quite some difficulties.
    What is the piece of paper that you get from a doctor or hospital with the description and results of your treatment called?
    What about the piece of document that you need (your personal doctor has to write and sign it) if you want to see a specialist doctor or to be accepted in a hospital?

    2) What about the piece of document that you need. . .
    In the UK, #1 is called the specialist's report.

    As for #2, our doctor makes a referral to a specialist, consultant or hospital and we receive an appointment soon afterwards.
