Suppose my car breaks down in a highway, I have to call AAA to tow my vechicle.
There are four possibilities of the location.
1) Close to exit 4, which means there is a short distance to access exit 4.
2) Just pass exit 4 a little,
3) Car died at exit 4 curve drive way(usually 0.1 mile long).
4) I tried to enter the highway from mileposter 4.(usually 0.1 mile long), the car died.

How to describe the location precisely in English?

I don't understand whether you have successfully driven onto the highway or your car has broken down on the sliproad (BrE). The first 3 say that you are either close to or just past exit 4, but number 4 says you tried to get onto the highway using mileposter 4, but the car died.

As a BrE speaker, my terminology is different, but when I call a breakdown firm, I have to say:

Hi, my name is XXXXX. My membership number is XXXXXX. My car has broken down. I am on the M25 (that is the name of a motorway), about a mile past Junction 12. Please come as quickly as possible.

My car has broken down on the A27 about halfway between Falmer and Lewes.

My car has broken down on Edburton Avenue, near the corner of Ditchling Rise.

No. 4 is different, I don't know what is the best term to describe the curve road. I would prefer America English because I am in USA.
I listened the radio NPR, the broadcaster said car died "approaching Exit 4" for case 1. I am not sure how about case 2.

"Approaching Exit 4" seems like the ideal way of saying it if the car died shortly before reaching Exit 4.

I am right on the I-95 southbound off ramp for exit 14.

I'm on the on-ramp from Route 322 in Chester going onto I-95 northbound.

I am on I-95 northbound, just past the on-ramp from 322 in Chester.
