It is clear that the second speaker has not fixed the TV. Therefore we can only have 'had intended' (or, possibly, 'intended') in the second gap. That leaves A or B as possibilities. 'Would' in the first sentence means that there must be a verb in a past tense in the first gap - A or D.
So, the answer must be A.
