Commas before conjunctions
Is it correct to have a comma before "and" / "or" in the following context:
"This includes demonstrating and communicating appropriate standards of workplace conduct, monitoring such conduct, and promptly reporting any alleged incidents or concerns to the next level of management, and/or our HR/Office Manager."
"Prohibited harassing conduct may include inappropriate jokes or innuendos; epithets, slurs, or negative stereotyping; displays in the workplace of offensive materials; and any other physical or verbal conduct or visual material."
Is it correct to have a comma before "and" / "or" in the above sentences?

The comma before "and/or" in the first sentence is wrong.
