Default "then" or "and then"?

Hoping someone here can give me a definitive answer on this one, preferably citing some authoritative source. Which of the following is correct (or are both acceptable)?

1. "He did this, then he did that."

2. "He did this, and then he did that."

I was trained to go with option 2, but my new supervisor keeps editing
things to option 1.

Well, the "and" is a bit redundant -- it doesn't add anything to the sentence.

I'm interested to know who "trained" you to use "and then". I had it drummed out of me at school: anyone writing "and then" in their essays was publicly hanged.

I didn't have any problem with the simple 'then' until MS Word suggested that I use 'and then' instead.
