Default writing a formal letter

hello, I'm in big trouble. i've just received a letter from my college inform that I had been withdrawn from the course because of poor attendence ( due to some personal problem, I didn't attend some recent class). they tell me if I want to appeal against this decision, I have to write a letter stating the reason for my appeal against withdrawal. I am really worry and confused. I don't know what I have to do. I'm an oversea student and I've just been here for a few months. I have never write a formal letter in English. Can you please help me. I'm very appreciate. thank you very much.

Paragraph one- state your purpose plainly and clearly- if you are going to appeal then say this, but don't give any further information
Paragraph two (and three if necessary) give the reasons for this appeal- the personal reasons, and any supporting evidence.
Last paragraph- state what action you want- thank them for their time and express a wish to find a satisfactory resolution to the issue
