Literally, "speaking through a hole in his neck" refers to someone who has had surgery (usually due to cancer) to remove his voicebox. The patient can no longer force air through his mouth to speak, and must undergo therapy to learn to speak through the surgical hole, or "stoma."

To pull Britain's fat out of the fire means to stop fueling whatever discord is going on, or to "rescue" it from a particularly heated situation.
