Dear Teacher,

What tense should I use with this kind of sentence?

1. This certificate was issued on January 12, 2002.
2. This certificate is issued on January 12, 2002.
3. This certificate has been issued on January 12, 2002.

I don't see much diference between #1 and #3.

#1 and #3 would be used if the certificate had already been issued (past)

#2 would be used if the certificate will be issued at a time in the future (Jan. 12th)

In most cases, one would use the past tense, #1.

This letter of certification is to certify that
I, the Doctor conducting an examination of the person named above, having performed a physical checkup on the following person:
Name-family name: Mr. A. A.A, aged 70 years
Identity card No. (specify the type of card): patient ‘s identity card of BB Hospital No. 751-40
On 3rd. March 2003.

From the physical checkup conducted on the person named above, the result found as follows:
The person I have examined
Physical condition: Is not the disable person of a kind that (to the extent that) is unable to perform the duty.
Metal condition: No symptom of insanity or insane delution or handicapped (mental retarded) and no symptom appearance of hazard habit-forming drug addiction and symptom of chronic alcoholism.
No symptom appearance or signs of the following diseases:
1. Leprosy in the communicable stage or the prodromal stage creating the social aversion.
2. Tuberculosis in the dangerous stage.
3. Elephantiasis in the stage of prodromal stage creating the social aversion.
(If it is necessary to examine for the disease related to the examinee ‘s functioning of the work, specify it in item 4).
4. AIDS.

Summary of doctor’s opinions: having a good and strong health. The patient has undergone the medical examination at BB Hospital from 19th. January 1997.
This certificate is issued on 3rd. March 2003.
I hereby certify that the above statement is true.
