It's usually used when someone is explaining something very complicated and the listener gets confused. Think of someone leading you through a maze that you don't know: if they go too fast, they may disappear around a corner and then they have lost you.

"What's a black hole?"

"Oh, it's quite simple. It's a star which has gone supernova, but the star is so massive that when it collapses, all the atoms' electron shells are shattered, the electrons fuse with the protons to give neutrons, then the neutrons are crushed to almost zero volume. At the point where the star's escape velocity exceeds the speed of light--"

"Sorry, you've lost me. Could you explain it a bit more slowly?"

It can also be used when someone says something that is very short, but still very confusing.

"What is the answer to the great question of life, the universe and everything?"

"Forty-two."

"Sorry, you've lost me there. Did you say, 'Forty-two'?"
