Default natives and TOEFL

I wonder if TOEFL reflects ones proficiency , and if a native takes that exam , should he get a high score even in the first attempt??

Some native speaker, if they took the exam without any knowledge, mind find some of the tasks difficult because of unfamiliarity with them. I have tested this with other advanced exams on native speakers and seen that some have found tasks in the Cambridge Proficiency exam difficult.
