Cool the restaurant is not sanitary
Can I say like the restaurant doesn't care about sanitary things? or do you have any other suggestions?

The restaurant is unsanitary.

You could then add a sentence explaining why you say this; do the workers not wash their hands? Are there insects in the building? etc.
