What is verb 'to be' and the verb 'be'?(same question with the verb 'to do' & 'do')

I am quite confused with this topic.

They are simply different forms of the same verb:

Be is the dictionary form and the bare infinitive (infinitive without 'to') form of the verb.
To be is the infinitive form.

Do is the dictionary form, the bare infinitive form, and the simple present form (except for 3rd person does) of the verb.
To do is the infinitive form.

So, infinitive is the simple present tense of a verb...

Is that all?....I think there are also 'had' and 'have',right?and also are there any first,second,third person forms acting here in the verb 'be'?
