Double indemnity clauses are found most often in life insurance policies. In the case of the accidental death of the insured, the insurance company will pay the beneficiary of the policy twice its face value. http://www.answers.com/topic/double-indemnity
I - "hung up but restrained herself" - Norma is talking to someone on phone
and wanted to hang up (disconnect, end the conversation), but
restrained (controlled) herself, and did not do so.
III - As she disconnected the phone, or ended her conversation, she
remembered Arthur's life insurance policy.
IV - To feel that this is not really happening, or as if in a dream.
You have included almost no context, which makes figuring out such things especially difficult. However, "she almost hung up but restrained herself" means that she considered hanging up but decided not to.
