I want to apply for a position and I don't know the meaning of two things, namely:

1. "Date appointed to present post or date promoted, if applicable."

2. "Period of notice required by employer." (Does it mean that they want to know how long before my leaving the company I have to inform my employer?)

1. "Date appointed to present post or date promoted, if applicable."

In other words, when did you start that job or, if you were promoted, when was that?

2. "Period of notice required by employer."

In other words, how many days' notice does your present employer require you give before you leave the company? In North America, 2 weeks' notice tends to be the norm.
