Default correct expressions

Hi,

could you please check the following senteces and especially the expressions in bold and word order:

1) This certificate was definitely taken into account when deciding on future cooperation.

2) Mr. XY submitted a form entitled “Supplier Approval” issued by the U.S. firm XY to us upon the first personal contact with your company, now our long-term partner, in our company in November 2001.
